The Pharmaceutical Care Network Europe (PCNE) was established in 1994  and it became an official association (under Dutch law) in 2004.
Pharmaceutical Care is the pharmacist's contribution to the care of individuals, in order to optimize medicines use and improve health outcomes.

Description

The thesis reviews concepts relevant to the delivery and outcomes of pharmaceutical care (including drug therapy problems), on the basis of which a conceptual framework for measuring the quality of medication use is developed. The framework is applied to develop an instrument to retrospectively measure the quality of medication use, which is subsequently field tested as an instrument to support continuous quality improvement of medication use in community  (Netherlands), inpatient (Germany) and hospital outpatient (Scotland) settings
