The Pharmaceutical Care Network Europe (PCNE) was established in 1994 by a number of European pharmaceutical care researchers. It became an official association (under Dutch law) in 2004.
During the PCNE Working Symposium in Hillerød, a PCNE definition was formulated for the term 'Medication Review'. On 20 february 2016, the definition was approved by the PCNE members in the General Assembly, which now makes it the official PCNE definition.
The PCNE definition of Medication review reads: 'Medication review is a structured evaluation of patient‘s medicines with the aim of optimizing medicines use and improving health outcomes. This entails detecting drug related problems and recommending interventions.'
A position paper for the definition will be published soon, and a scientific paper will be prepared on the process used to reach this consensus definition.
