The Pharmaceutical Care Network Europe (PCNE) was established in 1994  and it became an official association (under Dutch law) in 2004.
Pharmaceutical Care is the pharmacist's contribution to the care of individuals, in order to optimize medicines use and improve health outcomes.

Arterial and venous thromboses are important diseases associated with significant morbidity, mortality, and costs. Pre-filled syringes with heparins constitute one of the fastest growing markets in the drug delivery sectors. Reasons for handling difficulties and drug use problems are diverse and may lead to dosing errors or non-compliance. Pharmaceutical care aims to develop an individualized, patient-centered, and goal-oriented treatment plan to optimize safety and effectiveness.
