Report 2nd Medication review symposium, Dublin 2011
The Second PCNE Medication Review Working Symposium was held on the 18th October 2011 in Dubln, Ireland.
Apart from presentations about the status of medication review in different countries, and across different platforms, there were two major topics: the PCNE guidelines for medication review. and a coordinated research project about medication review across Europe.

Are the current guidelines up to date? (a draft can be found here). Is there a need to include a preliminary stage of prescription validation, or is that prospective medication review? What would optimally constitute stage 2: Pharmacy information + patient information or pharmacy information + medical information?

The discussions resulted in a preliminary statement about medication review, that will be proposed to the PCNE general assembly as the PCNE viewpoint on Medication Review.
PCNE Working group on medication review
In 2009 a PCNE Working Group on Medication Review was estabished in Vimeiro, Portugal. The working group is currently chaired by Dr. Nina Griese, Prof.Dr. Kurt Hersberger, and Dr. Foppe van Mil.
Medication review is a technique to detect drug-related problems in the medication of individuals. There can be several approaches: retrospective, prospective or ad hoc. Alle approaches have their specific challenges. Which appraoch can be chosen depends on the aim, the available expertise and available information. As for the latter, in a hospital setting the available information usually will differ from the community setting. The PCNE working group is in the process of developing pathways, protocols and guidelines for different types of medication review. The current draft version of the Guidelines can be downloaded.
The official PCNE definition:
Medication review is an evaluation of patient‘s medicines with the aim of managing the risk and optimizing the outcome of medicine therapy by detecting, solving and preventing drug-related problems.

March 2011: Medication review Mini-Symposium
On Saturday 26th March 2011, directly after the working conference of PCNE, a mini symposium on medication review was held in Manchester under supervision of the PCNE Medication Review Working Group.
The presentations held during this symposium can be found on the site of the Working Conference 2011. During this symposium, the definition of a Medication Review was revised.
August 2010: Medication review workshop
During the 2010 International Social Pharmacy Workshop (ISPW) in Lisbon, a workshop was held on "Medication review, the PCNE way". Materials from this workshop are available through the PCNE secretariat.
PCNE Working Symposium on medication review 2009
The the definition of medication review, and the different approaches have been extensively discussed during an PCNE Working Symposium on Medication in Geneva, on the Tuesday 3rd November 2009. There were 36 experts present who discussed definitions, the three types of medication review and the information that is necessary for performing medication reviews.
