Pharmaceutical
Care is defined by Hepler and Strand as the responsible
provision of drug therapy to achieve definite outcomes to
improve a patient’s quality of life.
Pharmaceutical
Care is a health care need associated with any drug therapy
and is a shared responsibility for professions in contact
with drug therapy.
Thus,
Pharmaceutical Care is a responsibility for pharmacists and
a key area in developing the professional role based on
health care needs. It is included as a part of Good Pharmacy
Practice, and it was the theme for a WHO conference in Tokyo
1993 and is by the conference and by WHO recommended as an
important role for the pharmacist.
Projects
should comprise the above in the basic concept mentioned
qualities to be considered as Pharmaceutical Care projects
in the PCNE frame.
